The following FEAR FACTOR list is a rating scale I use with my recovering anorexic and bulemic clients, who I love dearly. We come up with meal plans using all 1's and 2's to start, and gradually add a 3 once or twice a week. As confidence grows, the variety and "Safe" foods grow which is so wonderful to see...for both of us. Also consider that sometimes some foods may be 1's or 2's but too many together may be too much quantity and not be as "safe", so it'll be a higher number, but only as a meal.
